Forms that are not presently linked will be updated for the 2009 4-H year and available after January 1, 2009.

Ohio State Fair Representative PAS Form - Due May 1. Use this form to choose the classes in which you wish to qualify to represent Licking County at the Ohio State Fair.

Permission to Participate and Code of Conduct Form - Both forms need to be signed and returned to the Extension office by April 1 with member enrollment forms. All members taking horse projects, including Horseless Horse (#173), and their guardians are required to sign and return these forms. Cloverbuds in horse clubs are not required to submit these forms.

Identification for 4-H Horse Project - Horse ID paper is due on May 1.

Horseless Horse Mentorship Form - Due May 1 with Horse ID paper.

Horse Lease Form & Guidelines - Due May 1 with Horse ID paper. Click on link for leasing guidelines (purpose, expectations of owner and participant, and what items should be included in the lease). A sample lease is also included.

4-H Horse Show Category Confirmation - The club advisor is to turn this form in by May 1 accompanied by the club members' Horse ID papers.

Production Horse Essay Contest - Two lucky 4-H members will be awarded an Appaloosa filly and a Breeding Stock American Paint Association filly. Essay deadline is July 25 at 4:30 p.m. to the Extension office. Click on link for guidelines.

The Beeson Award "Outstanding 4-H Member" - Due to the Extension office by no later than 4:30 p.m. on Friday, August 29. Click on link for guidelines and form.

    Licking County 4-H Horse & Pony Council

    2008 Supplement - Click on link for the supplement that should be attached to the Uniform Rules for 4-H Horse Shows (#179). These are the rules under which all Licking County 4-H Horse Shows shall be conducted in Licking County.

    The next meeting of the Licking County 4-H Horse & Pony Council will be held on Monday, September 8 at 7:00 p.m. at the Licking County Extension Office.
