Member Record Form:

Use this form to maintain a record of your 4-H activities, projects, awards and honors. It will come in handy when you become eligible to apply for county and state achievement awards, scholarships, camps and conferences through the Ohio 4-H Achievement Record (see below). Click here for the member record form.

Ohio 4-H Achievement Record:

Apply for county and state achievement awards, scholarships, camps and conferences using the Ohio 4-H Achievement Record. Due in mid-October of each year, members who are 14 years old as of January 1 of the current year are eligible to apply. This packet will be posted early in September.

Project-related Opportunities:

  • Miami Valley Shooting Grounds 4-H Shooting Competition - May 31 starting at 9:00 a.m. at the Miami Valley Shooting Grounds in Vandalia, OH. Pre-registration due May 17. Registration also day of event. Click on the link for complete information and registration packet.

  • 4-H Dairy Foods Contest - Monday, June 23 at 6:30 p.m. at Zerger Hall. Click on link for complete information, map and recipe form.

  • Ohio 4-H Shooting Education Camp - July 13-18 at Canter's Cave 4-H Camp in Jackson, OH. For ages 12-18. Application & fee of $250 due by June 15. Fee increases to $295 after June 15.

  • Ohio 4-H Junior Shooting Education Camp - July 25-27 at Canter's Cave 4-H Camp in Jackson, OH. For ages 9-12. Application & fee of $150 due by June 20. Fee increases to $200 after June 20.
